Application Form
Please print out this page for easy reference. Next, from your Web
Browser, print the Application
Form. Once you have printed the Application Form, follow the steps
outlined below and completely fill in all requested information.
Part A: When are you planning to
take the MCAT?
Will it be during the spring
(e.g., January, March, April, May, June) or during the summer (e.g.,
July, August, September)? Check one box only and indicate the year.
Have you taken the MCAT before? Check either no or yes. If you answered
yes, what were your scores for the Physical Sciences (P), Verbal Reasoning
(V), Writing Sample (W), and Biological Sciences (B) sections of the exam?
Have you taken an MCAT review program before? If yes, what is the review
program's name?
Part B: Applicant's
Information.
Please fill in all the
information required in this section. Include a home phone number (no
pagers) and an E-mail address. Please print clearly. Your confirmation of
course enrollment will be sent to you via the US Postal Service (i.e., snail
mail) or by E-mail.
Part C: The Berkeley Review's
Locations.
Where would you like to take The
Berkeley Review? Check one box only.
Consult the Class Schedule.
Choose a Session that BEST fits your own schedule. Indicate this session
in the appropriate space. Be sure to indicate two (2) alternate review sessions
in the space provided (in order of preference) in case your first choice is not
available.
Do you want to begin with the Early Start program?
Check either no or yes. Please see the Class
Schedule for times and locations. If there is more than one Early Start
section for your course location, please indicate which one you would like.
Part D: Course
Tuition.
EARLY Enrollment and full payment BEFORE October 1st (if you
are taking the January exams), BEFORE December 1st (if you are taking the March, April,
May, or June exams), or BEFORE May 1st (if you are taking the July, August, or
September exams).
The full course tuition for The Berkeley Review is
$1,595.00. If you elect not to pay the full course tuition at the same
time as you enroll, a deposit of $895.00 is required in order to reserve
the session which you have indicated in Part C. The remaining balance of
$700.00 is due on or before October 1st if you are taking the
January exams, December 1st if you are taking the March, April, May, or June exams, or
May 1st if you are taking the July, August, or September exams. If full payment
is not received by the due date, a late fee of $30.00 will be
added. Please be aware that it is your responsibility to make sure
payment is made on time. Late fees are non-refundable.
LATE Enrollment and full payment AFTER October 1st (if you are
taking the January exams), AFTER December 1st (if you are taking the March, April, May,
or June exams), or AFTER May 1st (if you are taking the July, August, or
September exams).
The full course tuition for The Berkeley Review is
$1,625.00. If you elect not to pay the full course tuition at the same
time as you enroll, a deposit of $925.00 (includes a $30.00 late
fee) is required in order to reserve the session which you have indicated in
Part C. The remaining balance of $700.00 is due by the first class
session. Additional late fees will apply if payment is made after the first day
of class. Late fees are non-refundable.
REPEAT Enrollment.
If you have taken The Berkeley Review
before, you must call the main office in Berkeley at (800) 622-8827 and speak to
a representative about your status BEFORE you submit the Application Form.
Payment.
Checks are made payable to The
Berkeley Review. All fees must be paid in full by the first class
sesion, or you will be dropped from the course.
A $125.00 non-refundable administrative fee is included in
the full course tuition and/or deposit. The minimum fee for a
check which is not honored by the bank on which it is drawn is $50.00.

Part F: All Applicants must read
the Enrollment Agreement and sign the Application Form.

Agreement. Make sure all parts of the Application Form are complete, sign and
